Job Description:

The Nurse Navigator, as a member of the patient care team, collaborates to coordinate service and ensure quality of care for patients in the infectious disease clinic areas. This nurse interdependently assesses, evaluates and implements care delivered to patients and ensures a smooth transition of patients from clinic to procedural and outpatient treatment areas. The nurse navigator, together with the multidisciplinary team, facilitates timely scheduling of appointments, diagnostic testing, and procedures to expedite the plan of care and promote continuity of care. The nurse navigator must possess the skills needed to engage in the nursing process including but not limited to assessment, nursing analysis, planning, nursing interventions, and evaluation, delegation, and maintenance of health across the continuum of life. This nurse must also be able to effectively and appropriately delegate and supervise completion of delegated tasks by unlicensed assistive personnel. The Nurse Practice Act and Administrative Rules of the state of Montana govern registered nursing practice. The role of the the Infectious Disease Nurse Navigator is to assist with care coordination of infectious disease patients scheduled for discharge and also assist with the transition to ongoing care in the outpatient setting. The Infectious Disease Nurse Navigator also tracks and coordinates care for long-term patients of the ID service. This is accomplished by direct patient care in the clinic, reviewing medical records, interviewing the patient (by phone or in person), reviewing and/or obtaining tests and consultations as indicated. Providing education to patients and their caregivers. The role of the Infectious Disease Nurse Navigator also involves tracking of data related to patient care and outcomes and utilizes this information to improve clinic workflows, delivery of patient care, patient compliance, patient safety and efficiency of the care team.REQUIREMENTSEDUCATIONRequired:Bachelors of Science in Nursing from accredited nursing schoolPreferred:LICENSURE and CERTIFICATIONRequired:Current unrestricted Montana RN License Current AHA Health Care Provider BLSPreferred:Ambulatory Department certifications preferredEXPERIENCERequired:1 year of direct patient care with exceptional assessment skillsPreferred:Infectious Disease, Critical Care or Oncology/Infusion Experience, Ambulatory clinic experience or Nurse Navigator Experience KNOWLEDGE OF, SKILLS IN, ABILITY TO, COMPLEXITY AND DIFFICULY:KNOWLEDGE OF:Perform activities that demonstrate nursing accountability.Nursing best practices and efficiencies. Basic knowledge of pathophysiology as it relates to acute and chronic disease processes.Knowledge of various diagnoses and treatments of disease states and knowledge of wellness/health maintenance.SKILLS IN:Strong critical thinking, clinical assessment, and nursing clinical practice skills.Utilize computer skills - hardware basics and software programs including e-mail, word processing, and spreadsheets.Demonstrate customer service skills including interpersonal, verbal, and written communication skills.Organizational and time management skills.Efficient in the use of electronic health records.ABILITY TO:Perform activities that demonstrate nursing accountability.Consideration of factors related to safety, effectiveness, and cost planning when delivering care to patients. Prioritizes duties and use of time while assisting in navigating patient through the clinic in a friendly, professional, and efficient way. Documents in a legally defensible manner in the medical record all phone conversations, verbal instructions, medication administration and any other items discussed with patients, their families, and other individuals involved in the patient's care. Demonstrate initiative in performing responsibilities. Demonstrate ability to pay close attention to detail in providing patient care. Assists with orientation of newly hired nursing staff.Maintain flexibility to adapt to a variety of work assignments.Work independently with a minimum of supervision.Education of patients and their care providers.Follow established policies and procedures and refer to policy manual for specific instructions/guidelines. Adhere to the standards of the job description and overall philosophy of Bozeman Health Deaconess Hospital. Maintains yearly mandatory education requirements per department standards.Maintains current Basic Life Support.COMPLEXITY AND DIFFICULTY:ESSENTIAL FUNCTIONS15% Time Spent -Coordination of infectious disease clinic patients: Using risk stratification, identify and assess patients that would benefit from complex care management.20% Time Spent - Actively participates as a team member by facilitating collaboration and communication between providers, schedulers, nursing, infusion, radiology, lab, and other departments or community resources encouraging seamless access to care. Work with an inter-disciplinary team to develop individualized plans of care to promote health and provide support, education, and resources for illness and wellness management. 20% Time Spent -Gather and track data related to key performance indicators and patient outcomes indicators. Continually analyze data for clinic development ideas, improvement in patient outcomes, and improvement of quality data. Assist in tracking of incidental diagnostic findings as needed. Contact patients if needed to assure appropriate follow up. 10% Time Spent - Coordinate the ordering, scheduling, and performance of selected procedures and treatments as indicated. Provide patient education and instruction regarding procedures and treatments as indicated. Provide ER, hospital, and nursing home follow up calls, schedule appointments and coordinate with other specialties to arrange appointments, provide medication review and disease-related education. 10% Time Spent -Assess for and provide nursing interventions for actual or potential problems that the patients and family may experience related to the disease process and treatment. 10% Time Spent - Analyze assessment data to determine nursing diagnoses in order to identify expected outcomes individualized to the patient and their family and facilitate smooth transitions in care.Facilitates individualized care based on culture, health, literacy, ethics, and psychosocial needs.5% Time Spent -Collaborate to provide patients with appropriate resources including but not limited to: behavioral health, substance abuse, wellness activities, social determinants of health (e.g. food insecurity, housing, financial, transportations), arranging access to community resources. Assist with Survivorship care planning as needed.5% Time Spent -Provide effective patient education that is appropriate for health literacy level including but no limited to: plan of care, diagnosis, resources, follow up care. Effectively communicate gaps in patient education to providers and other care team members. Works with non-licensed staff to gather necessary medical records for review.5% Time Spent -Engage in process improvement surrounding work flows and patient care processes within the department. Room patients - Gather patient data consistently and accurately, collect information on history of present illness or injury, obtain vital signs, review current medications and allergies. Document patient encounters in EMR/paper chart. Triage and respond to patient phone messages in a timely manner according to clinic guidelines, collaborate with provider, act on provider response, and communicate with patient; document task communications in EMR/paper chart. Serve as primary contact and intermediary between provider, patient, family, home caregivers, pharmacists, and hospital personnel and advocate for the patient between office visits or treatments. Other tasks and duties as assigned.
