Job Description:

The oncology nurse navigator, as a member of the patient care team, collaborates to coordinate service and ensure quality of care for patients receiving treatment in the Cancer Center. This nurse interdependently assesses, evaluates and implements care delivered to patients in the Cancer Center and ensures a smooth transition of patients from during active treatment and into survivorship or end-of-life care. The nurse navigator, together with the multidisciplinary team, facilitates timely scheduling of appointments, diagnostic testing, and procedures to expedite the plan of care and promote continuity of care. The Nurse Practice Act and Administrative Rules of the state of Montana govern registered nursing practice. The oncology nurse navigator must possess the skills needed to engage in the nursing process including but not limited to assessment, nursing analysis, planning, nuring interventions and evaluation, delegation, and maintenance of health across the continuum of life. Participates as a care team member with the physician in the assessment, planning, implementations, and evaluation of patient care needs. 5% Time SpentParticipates in tracking and documentation of metrics and patient outcomes in order to evaluate outcomes of the navigation program.20% Time SpentProvide patient education that is appropriate for health literacy level including but not limited to: plan of care, diagnosis, resources, follow up care. Effectively communicates gaps in patient education to providers and other care team members.10% Time Spent Facilitates individualized care based on culture, health, literacy, ethics, and psycho-social needs. Serve as primary contact and advocate for the patient between office visits or treatments. Provides direct patient care, which encompasses teaching, symptom management and monitoring physical, psychological, social and educational needs. 10% Time Spent Assesses and provides nursing interventions for actual or potential problems that the patients and family may experience related to the disease process and treatment.10% Time Spent Analyzes assessment data to determine nursing diagnoses in order to identify expected outcomes individualized to the patient and their family and facilitate smooth transitions in care. 5% Time Spent Serves as an intermediary between the provider and patients, relatives, home caregivers, pharmacists and hospital personnel.10% Time Spent Coordinate scheduling of appointments, referrals, diagnostic procedures and track the outcomes of such. Assists with scheduling and coordination of care for all diagnostic testing, follow up appointment, and referral appointments.10% Time SpentResponds to in basket messages and telephone calls in a timely, efficient, and appropriate time frame. 10% Time Spent Provides patients with test/procedure/referral instructions and information as requested by provider. Partners with the patient and family, the inter-professional team, and community resources to optimize patient care.20% Time Spent Collects Comprehensive data pertinent to the patients health or the situation (Assessment) Collects data in a systematic and ongoing processInvolves the patient, family and other healthcare providers in data collectionPrioritizes data collection activities based on
