Join our team today as a Registered Nurse Case Manager to positively impact the experiences of patients and families.Position SummaryAs a member of the Inter-Disciplinary Team, the RNCM will work as a hospice field nurse to provide both routine and emergency assessments as well as evaluative services to meet the needs of the patient and their family. You will help maintain their maximum level of comfort and coping, flex your schedule to meet agency-staffing needs, and provide after-hours emergency support to the patient and family.Principal Accountabilities



    • Coordinate clinical, psycho-social and spiritual services as indicated by Plan of Care through case management.

    • Provide physical and emotional intensive care to support the patient and family in periods of crisis.

    • Collaborate with the Inter-Disciplinary Team to coordinate hospice care for the patient and family to ensure appropriateness, continuity, and quality of care.

    • Explain hospice services and Medicare benefits to patients and families.

    • Oversee the care given by Licensed Practical Nurses/Licensed Vocational Nurses and Hospices Aides.



      Qualifications



      • Strong leadership and organizational skills required.

      • Effective communication and compassionate interpersonal skills.

      • Knowledge of Microsoft Office programs.

      • A valid driver's license and auto liability insurance.



        Education and CertificationsRegistered Nursing License in state of employment. Minimum of one year nursing practice or equivalent experience. Position SummaryThe Chaplain/Spiritual Care Coordinator is a hospice-trained individual who provides spiritual support to patients and families. As a Chaplain/Spiritual Care Coordinator, you may be a priest, minister, rabbi or other individual with experience in spiritual support; your role is not intended to replace the patient's own clergy, but to bring spiritual guidance and support.Principal Accountabilities



        • Explains hospice services and Medicare benefits to patients and families; obtains Informed Consent and Election of Benefits documents as requested.

        • Submits appropriate documentation and paperwork to facilities at the completion of patient visits; documentation is completed and submitted timely according to company policy.

        • Articulates the regulatory framework governing the role of the Chaplain/Spiritual Counselor (federal and state, if applicable).

        • Reads and incorporates into practice the policies and procedures for Compassus in the provision of care and services to hospice patients.

        • Participates as a member of the IDT, including development and implementation of the plan of care.

        • Provides spiritual care support to the patient and family; supports the involvement of community clergy as the patient/family request.

        • Assists in identifying the need for intervention of other IDT members.

        • Effectively communicates patient and family needs to IDT.

        • Works collaboratively in a team environment to promote the values of hospice and of Compassus.

        • Collaborates with IDT to coordinate spiritual care support for the patient and family to ensure appropriateness, continuity, and quality of care.

        • Maintains updated, comprehensive care plans which reflect current problems, goals and interventions for patients.

        • Maintains respect for the family's environment and belief systems, and remains nondiscriminatory regarding age, race, religion, sex, sexual orientation, national origin, physical or mental disability, and other personal matters.

        • Counsels in accordance with patient's and family's beliefs.

        • Supports the patient and family in working through the stages of death, grief, and bereavement.

        • Acts as liaison with the clerical community to interpret the needs of the hospice patient and family, and helps recruit clerical volunteers for the hospice.

        • Promotes hospice philosophy.

        • Represents Compassus with professionalism, integrity and competence.



          Education and/or ExperienceMinimum of bachelor's degree in counseling, philosophy, divinity, theology or similar field required; Clinical Pastoral Education a plus. Ordination in an organized religious group preferred. Minimum 1 year of experience in the provision of spiritual counseling support to individuals and families required.State-specific certificationsGeorgia: Documentation of ordination or commission by a recognized faith group; completed at least one unit of clinical pastoral education from a nationally recognized provider.Louisiana: Bachelor of Divinity, Master of Divinity or equivalent theological degree or training.Missouri: Documentation of ordination or commission by a recognized faith group and has completed one (1) unit of Clinical Pastoral Education (CPE)Mississippi: Bachelor of Divinity, Master of Divinity or equivalent theological degree or training.Mathematical SkillsAbility to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Language SkillsAbility to understand, read, write and speak English. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or colleagues of organization. Ability to effectively present information and respond to questions one-on-one and from groups of patients, families or colleagues.Reasoning AbilityAbility to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or schedule form.Computer SkillsTo perform this job successfully, an individual should be proficient in Microsoft Office.Other QualificationsUnderstanding and acceptance of hospice philosophy. Must be of high integrity including maintenance of confidential information including all patient records. Good communication skills. Ability to work a flexible schedule, to include some evenings and weekends.Certifications, Licenses and RegistrationsClinical Pastoral Education preferred or required per state regulation. Driver*s license and auto liability insurance required.Other Skills and AbilitiesStrong skills in counseling the terminally ill and his/her family. Strong organizational, interpersonal, and communication skills. Must be able to travel. A positive, compassionate, and empathetic attitude.
