Job Description:
Intermountain Healthcare is seeking a dedicated and
compassionate Social Work Care Manager to join our team. As a
pivotal part of our healthcare services, the Social Work Care
Manager will assess patient needs, coordinate care plans, and
provide advocacy services to ensure optimal health outcomes. This
role requires a deep understanding of patient care, the ability to
navigate complex healthcare systems, and a commitment to delivering
the highest standards of health services. Conduct comprehensive
assessments of patient social, emotional, and environmental needs.
Plan and implement care strategies that are individualized and
culturally competent. Collaborate with healthcare providers,
external agencies, and community resources to facilitate access to
services. Monitor and evaluate patient progress, revising care
plans as needed. Provide crisis intervention and support services
to patients and families. Advocate for patient rights and services,
ensuring equitable healthcare delivery. Strong interpersonal and
communication skills Ability to work collaboratively in a
multidisciplinary team Knowledge of clinical social work practices
and principles Proficiency in case management software and medical
records systems Expertise in crisis intervention techniques Medical
Insurance Dental Insurance Vision Insurance Life Insurance 401k
Licensed Clinical Social Worker (LCSW) certification Additional
certifications in mental health services or geriatric care are
advantageous
