+ POSITION DESCRIPTION+ POSITION SUMMARY+ The Perioperative Surgical Home Nurse Navigator collaborates with providers and/or clinical areas to coordinate service and ensure quality of care for patients preparing to have a surgical or procedural service. This nurse interdependently assesses, evaluates and implements care delivered to patients in Perioperative Surgical Home and ensures a smooth transition of patients from pre-procedural. The nurse navigator, together with the multidisciplinary team, facilitates timely scheduling of appointments, diagnostic testing, and procedures to expedite the plan of care and promote continuity of care. The registered nurse also plans and implements patient and family education. The Nurse Practice Act and Administrative Rules of the state of Montana govern registered nursing practice.The role of the Perioperative Surgical Home Nurse Navigator is to assess the health and readiness of patients scheduled to undergo anesthesia-requiring procedures. This is accomplished by direct patient care in the clinic, reviewing medical records, interviewing the patient (by phone or in person), reviewing and/or obtaining tests and consultations as indicated. Provide education to patients and their care givers.+ REQUIREMENTS+ EDUCATION+ Required:+ Bachelors of Science in Nursing from accredited nursing school+ Preferred:+ LICENSURE and CERTIFICATION+ Required:+ Current unrestricted Montana RN License+ Current Health Care Provider BLS+ Preferred:+ Ambulatory Department or Surgical Related certifications preferred+ EXPERIENCE+ Required:+ 3 years of direct patient care in Surgical Services or Clinic Care with exceptional assessment skills+ Preferred:+ Experience in Perioperative, Surgery, Education, and/or Quality areas+ KNOWLEDGE OF, SKILLS IN, ABILITY TO, COMPLEXITY AND DIFFICULY:+ KNOWLEDGE OF:+ + Basic knowledge of pathophysiology as it relates to surgical procedures. + ASA classification, Pre-Surgery testing guidelines, acquiring personal health histories from patients + Knowledge of different types of surgeries from various specialties (Orthopedic, Urology, Spine, ENT, General, OB-Gyn, Opthalmic ,Endoscopy etc.) + Personal Computers, hardware and basic software programs including e-mail, word processing, and spreadsheets.+ SKILLS IN:+ + Strong nursing clinical practice. + Nursing best practices and efficiencies. + Organizational and time management skills. + Documentation of patient medical history + Advanced oral and written communication skills + Education of patients and their care providers+ ABILITY TO:+ + Refer to policy manual for specific instructions/guidelines. + Follow established policies and procedures. + Adhere to the standards of the job description and overall philosophy of Bozeman Health.+ Maintain flexibility to adapt to a variety of workload assignments+ Demonstrate initiative in performing responsibilities.+ Work independently with a minimum of Supervision.+ Is accountable to department and organization for following standards of care.+ Ability to work in a team environment and be attentive to detail while maintaining a positive attitude.+ Demonstrate integrity and is ethical in conduct and decision making+ COMPLEXITY AND DIFFICULTY:+ + Strong critical thinking and clinical assessment skills+ Proficient at interpreting cardiac rhythms.+ Maintains yearly mandatory education requirements per department standards + Maintains current Basic Life Support+ ESSENTIAL FUNCTIONS+ 85%+ + Performs pre-anesthesia/pre-procedural/surgical assessment with the patient/guardian. Conducts the interviews by telephone or in person. Gathers information necessary to perform informed interviews with patients which encompass teaching, assessing physical, psychological, social and educational needs. Facilitates timely scheduling of appointments, diagnostic testing, and procedures. + Actively participates as a team member by communicating with the Hospitalists, Anesthesiologists, Surgeons, Advanced Practice Providers, Clinic staff, and other departments, providing thorough documentation. Utilizes the American Society of Anesthesiology and American Heart Association/American College of Cardiology guidelines for non-cardiac surgery. + Works with non-licensed staff to gather necessary medical records for review. + Works with other co-workers to improve care processes in the department. + Room patients - Gather patient data consistently and accurately, collect information on history of present illness or injury, obtain vital signs, review current medications and allergies. Document patient encounters in EMR/paper chart. + Triage and respond to patient phone messages in a timely manner according to clinic guidelines, collaborate with provider, act on provider response, and communicate with patient; document task communications in EMR/paper chart.+ Actively participates as a team member by communicating with the Hospitalists, Anesthesiologists, Surgeons, Advanced Practice Providers, Clinic staff, and other departments, providing thorough documentation. Utilizes the American Society of Anesthesiology and American Heart Association/American College of Cardiology guidelines for noncardiac surgery.+ Develops a plan that prescribes strategies and alternatives to attain expected outcomes (Plan of Care)+ Develops an individualized plan of care considering patient characteristics or the situation+ Develops the plan in conjunction with the patient, family and others as appropriate+ Includes strategies within the plan that address each of the identified diagnoses or issues+ Provides for continuity within the plan+ Establishes plan priorities with the patient, family and others as appropriate+ Utilizes the plan to provide direction to other members of the healthcare team+ Implements the identified plan of care+ Implements the plan of care in a safe and timely manner+ Documents implementation and any modifications to the plan of care+ Utilizes evidence-based interventions and treatments specific to the diagnoses or problem+ Collaborates with nursing colleagues and others to implement the plan of care+ Coordinates the implementation of the plan of care+ Documents the coordination of care+ Employs strategies to promote health and a safe environment+ Provides health teaching that addresses such topics as healthy lifestyles, risk-reducing behaviors, developmental needs, activities of daily living and preventative self-care.+ Seeks opportunities for feedback and evaluation of the effectiveness of health teaching strategies used+ Systematically enhances the quality and effectiveness of nursing practice+ Demonstrates quality by documenting the application of the nursing process in a responsible, accountable, ethical manner.+ Uses the results of quality improvement activities to initiate changes in nursing practice+ Uses creativity and innovation in nursing practice to improve care delivery+ Incorporates new knowledge to initiate changes in nursing practice+ Participates in quality improvement activities, including but not limited to:+ Identifying aspects of practice important for quality monitoring+ Using indicators developed to monitor quality and effectiveness of nursing practice+
